Does anyone know how we might determine the correct
output level to drive the TX when playing back Wav files.
My CQ's sound great, but you cannot change the main meter
setting to Power Output or ALC during Wav playback. I just used
an outboard wattmeter and zero dB on the level setting on the Wave
menu seems to be close to correct (same output as a correctly modulated
voice signal from the mic)

You went to Wave menu in the toolbar and adjusted TX Gain to your needs (was 0 dB). I had to add 10 dB to get the same average output when playing a recorded wave file. Changing the TX meter mode works in receive or transmit even when playing back a file in my setup (Orion board on 200D Thetis v 2.6.69(1/25/20).

A tip. If you use Audacity to record test or voice keyer wav files, you can apply the amplify Effect to 0db with no clipping allowed and save the wav after that. I still had to trim the tx gain a bit on the thetis wav playback but a good bit less. For setup testing this was useful to get the initial mic level close to 0 to set up EQ and compression. After I was done, I just trimmed up with the TX gain level for the actual mic audio.
